\JI am returning the Enrollment Form which you sent us with respect to policy
number 473731037 under the name of Bernice D. Samuel.

Please note that we traded the '67 Chevrolet in on a 1977 Chevrolet as
of April 25.  You were informed of this by phone on April 26th and a
request made to increase the coverage on this new car to that provided by
Plan 55.

We were informed this morning by phone that this extended coverage is
already in effect but that we should return the Enrollment form as is.
Unfortunately I had already noted the car change on the form but I judge
that it will still be acceptable.  I was told that you would bill us for
the addittional cost and that it would not be necessary for us to return the
form notifying you of the car replacement.\.
